PRESS RELEASE – Hict joins IMPACT‑MED, a major European initiative advancing integrated, AI‑enabled cardio-metabolic care.

Belgium – April 2026 – Hict is proud to announce its participation in IMPACTMED (Improving patients’ outcomes & fragmented specialised care through a connected & advanced Multidisciplinary CardioMetabolic Clinic), a newly launched European project funded under the Innovative Health Initiative (IHI).

The consortium officially kicked off earlier this year, marking the start of an ambitious 60‑month programme aimed at transforming the management of cardio-metabolic diseases across Europe.

Advancing integrated care for cardio-metabolic diseases

Cardio-metabolic conditions, including hypertension, obesity, diabetes, coronary artery disease, and heart failure, often coexist and remain among the leading causes of death and disability in Europe.

IMPACT‑MED addresses these challenges by developing a scalable, peoplecentred Multidisciplinary CardioMetabolic Care (MCMC) framework, supported by a clinically enriched digital ecosystem combining advanced AI technologies and real‑world clinical data.

The project will enable more integrated, proactive, and personalised care pathways, powered by innovations such as AI‑based risk stratification, predictive analytics, remote monitoring, and patient‑centred data tools.

Hict’s leadership role in the project

Within this large‑scale European collaboration, Hict plays a central role as leader of Work Package 5 (WP5): “Implementation of MCMC in RealWorld Settings.”

Within this role, Hict will support:

  • The coordinated deployment of clinical pilot sites across Europe in collaboration with consortium partners
  • Real‑world validation and uptake of the MCMC framework across diverse healthcare settings
  • Monitoring of performance, implementation progress, and scalability to inform continuous improvement and wider adoption

This leadership positions Hict at the heart of IMPACT‑MED’s mission to translate digital innovation into practical, scalable improvements in patient care delivery.

A strong European consortium

IMPACT‑MED brings together 25 leading organisations from across Europe and beyond, including hospitals, research institutes, industry leaders, SMEs, and patient organisations. The consortium includes partners such as Medtronic, Siemens Healthineers, Samsung, Perspectum, CERTH, and major clinical centres like AP‑HP and Leeds Teaching Hospitals.

This multidisciplinary collaboration enables a unique combination of clinical expertise, digital innovation, AI, and patient engagement, ensuring a holistic approach to cardio-metabolic care transformation.

Towards a connected and patientcentred future of care

Through the integration of digital health technologies and coordinated care pathways, IMPACT‑MED aims to reduce fragmentation, improve patient outcomes, and support valuebased healthcare.

By validating its solutions in real‑world clinical settings and developing clear pathways for scalability and sustainability, the project will contribute to a future where healthcare systems are more connected, efficient, and truly centred on patients’ needs.
